		<description>My baby was born in a Canberra hospital in 2006 with facial bruising and lacerations from forceps due to malpractice. The birth trauma was not included in the birth summary or neonatal record. Luckily we have pictures of the damage to include in our report which clearly show the extent of her injuries. This, coupled with an incomplete maternal chart and  massive deviations from protocol and standard of care have spurred some very hard questions for those involved. 

Just because this survey details a decline in birth injuries does not mean there&#039;s been an actual decline in injuries, it just details a decline in REPORTED injuries.</description>
